Service Provider Contact Form
Service providers have ongoing maintenance and reporting requirements that ensure they are compliant with government regulations and regional center requirements. Per California Code of Regulations Title 17 Section 54332(b) regional centers shall review all vendor files biennially to ensure a vendor continues to be in compliance with vendorization requirements. Following is the various documentation vendors are required to submit along with guidelines. Please use the contact form to submit any questions regarding compliance.
Please note that the below requirements are conditions of doing business with the Center and failure to comply will result in a “do not refer” status and ultimately devendorization.
Annual
Insurance requirement: Lanterman requires all regional center service providers, whether vendored by Lanterman or another regional center, to maintain appropriate liability insurance and to name “Frank D. Lanterman Regional Center” as an additional insured on such policies.
Lanterman Regional Center has partnered with Gallagher Verify to assist with the collection, processing and tracking of insurance certificates. Gallagher Verify will begin reaching out to providers to request the certificates of insurance (COI). Effective 9/1/25, all COIs should be sent directly to GSC.COIR.Lanterman@ajg.com.
Lanterman along with all California regional centers, have a longstanding expectation that all vendored service providers that receive funding via a regional center Purchase of Service (POS) maintain certain minimum insurance requirements. The intent is to protect the interests of the individuals served by the Center, their families, service providers, and the Center itself, and to ensure a safe and healthy environment for all individuals served.
This is a condition of doing business with the Center and must take place at the time of vendorization. Additionally, we need a current Certificate of Insurance with our Center named as additionally insured each time you renew your insurance policy to satisfy this requirement. Lanterman requires the following:
- All service providers shall obtain and maintain General Liability insurance with a limit of at least $1,000,000.00 per occurrence and at least $3,000,000.00 aggregate annually.
- All service providers that provide direct care services and supports shall obtain and maintain Professional Liability insurance with a limit of at least $1,000,000.00 per claim and at least $3,000,000.00 aggregate annually.
- All service providers that provide direct care services and supports shall obtain and maintain Abuse and Molestation liability insurance policy with a limit of at least $1,000,000.00 per occurrence and at least $3,000,000.00 aggregate annually
- All service providers that have access to or control over assets of individuals served shall maintain bond insurance in a sum sufficient to cover the value of the individuals’ assets.

Certified Public Accountant (CPA) requirement: Per Welfare and Institutions Code (WIC) section 4652.5, regional center service providers that receive at least $500,000 or more from one or more regional centers in each state fiscal year need to complete and submit an independent review or audit to the regional center within nine months from the close of the provider’s fiscal year. Biennial
Per California Code of Regulations Title 17 Section 54332(b) regional centers shall review all vendor files biennially to ensure a vendor continues to be in compliance with vendorization requirements. As per those regulations the following actions are required:
- If services have not been provided to individuals within the past two years, the regional center will deactivate that service provider and notify local regional centers. Should service coordination request services of a deactivated vendor, FDLRC Community Services will reactivate the vendor in our system should the service provider comply with this regulation.
- If an FDLRC vendored service provider has provided services within 2 years of their vendorization (or another timeline set by Community Services) and intends to continue providing services that service provider will:
- Complete an updated Disclosure Statement DS1891 using this link.
- Complete a Biennial Self-Certification form which can be found here attesting to being in compliance with all initial vendorization regulations. For agencies with multiple vendorizations we require one DS1891 per vendor number.
- Failure to comply with these requirements will result in the service provider being placed on a Do Not Refer and ultimately devendorization.
It is a service provider’s responsibility to track the 2-year cycle in order to re-submit these documents.

Service Provider Organizational Changes
Per regulatory requirements (Title 17 CCF § 54330) and Lanterman expectations, please be reminded that Lanterman Community Services needs to be formally notified, meaning via a letter on your letterhead, within 30 days of the following organizational changes:
- Rate change
- Address change
- Change in ownership and/or administration
- Change in tax ID
- Change in required license, certificate, registration, credential or permit
Similarly, Lanterman Community Services needs to be formally notified within 60 days of the following changes:
- Vendor closure
- Program or service design change

How do I become a Lanterman service provider?
Service providers must be vendored by a regional center before they can provide and be reimbursed for services. Please see our “How to Become a Service Provider” page, which provides detailed instructions.
How can I make a change to the services I provide?
Any change to your existing vendorization needs to be reviewed and processed by Lanterman’s Community Services Unit.
- Organizational changes require submission of a letter in advance of the change consistent with the parameters and process found here.

How can I renegotiate my rate?
Rate setting rules are established both by the statute (Lanterman Act) and regulation (Title 17) and dictate the type of rate needed for a particular service type. Providers with negotiated/median rates or DDS set rates can adjust rates based on minimum wage ordinances, Health and Safety Waiver needs or statutory/regulatory changes. Providers with Medi-Cal set rates, can see rate changes consistent with Medi-Cal rate changes. Usual and Customary Services providers can request rate adjustments consistent with changes to their general fee schedule. What is FDLRC’s service provider insurance requirement?
All service providers, whether vendored by Lanterman or another regional center that serve Lanterman individuals, must carry liability insurance and name Frank D. Lanterman Regional Center as additional insured consistent with the following insurance memo. Additionally, updated insurance certificates must be submitted to Irma Padilla at ipadilla@lanterman.org.
What is a DS1891 and why do I need to resubmit?
The DS1891 Disclosure Statement is a Department of Developmental Services (DDS) form which must be completed at the time of vendorization and subsequently every 2 years moving forward unless requested by the regional center sooner. In short, this statutory requirement (WIC Code § 4748.12) does not allow individuals or organizations convicted of fraud, abuse or neglect to be vendored. Regional centers are required to monitor vendorization requirements biennially per California Code of Regulations Title 17 § 54332(b).
How can I get help with e-billing?
Every regional center service provider has an assignment Accounting Associate who can help with all payment and billing issues. You can find your assigned Accounting Associate here and by logging into your e-billing portal.
How can I let service coordinators know about my services?
At the time of vendorization, Community Services notifies all of service coordination staff of the newly vendored service provider. Capacity and availability can be communicated to Quality Assurance Manager Sonia Garibay at sgaribay@lanterman.org or Director of Community Services Pablo Ibañez at pibanez@lanterman.org. In turn, Community Services will notify service coordination of your services.
How do I find out who is the service coordinator assigned to the individual we are serving?
